Home Centered Care Institute

This grant supports the Cohesive Integration of Home-Based Palliative Care and Home-Based Primary Care in the ILHCP, helping providers incorporate palliative care into the Illinois House Call Project. The initiative will support training and resources that help clinicians establish or expand home-based palliative care programs, improve care coordination, and strengthen support for patients with serious illness receiving care at home. Activities include developing provider resources, offering educational programming and webinars, and creating tools for caregivers and clinicians to improve quality of care and sustain home-based palliative care services across Illinois.
