Northern Illinois Hospice Association

This grant supports the Northern Illinois Palliative Care Social Work & Community Education Initiative, which provides psychosocial support and resource assistance for patients with serious illness and their care teams. The initiative also expands community education through social work programming and outreach, helping patients, caregivers, and professionals better understand and access palliative care services. Activities include developing educational materials, expanding online resources, and offering training opportunities for social workers and care teams to strengthen support for individuals facing serious illness in Northern Illinois.
