National Request for Proposals 2025

Purpose

The purpose of this Request for Proposal (RFP) is to invite values-aligned non-profit entities, including educational institutions, to submit applications for funding to support various palliative and hospice care related activities. Through this grant program for 2025, Elea Institute anticipates awarding up to $3,000,000 in total awards to between five and ten organizations.  Areas of potential funding are built around Elea Institute’s  aspirational pillars:

Background

Elea Institute is a nonprofit charitable organization created out of the proceeds from the 2022 sale of JourneyCare, a nonprofit hospice agency that stood at the forefront of hospice and palliative care in the greater Chicago metropolitan area.  The organization’s name is inspired by the mythological figure Elea, who was the personification of mercy and compassion in ancient Athens.

The primary goal of Elea Institute is to honor our heritage and advance the missions that guided JourneyCare and its predecessor organizations: to ensure that all people have access to high-quality hospice and palliative care.

Elea Institute is dedicated to improving care and comfort for people with serious illnesses by awarding grants for innovative programs and initiatives, funding research, and advancing policies to promote high quality, widely accessible hospice and palliative care.

RFP Details

Through this RFP, Elea Institute anticipates awarding up to $3,000,000 in total awards to between five and ten organizations

  • Minimum grant size: $150,000
  • Maximum grant size: $1,000,000
  • Grant project period: up to 4 years
  • Maximum indirect rate is 20%

To apply for a grant, register and login to the Institute’s online application system. Proposals must be completed and submitted with all required responses and attachments in order to be considered for funding in this grant cycle. Please use the access code “Elea25” to access the application.

  • RFP released – June 16, 2025
  • Letters of Interest due – July 11, 2025
  • Applicants notified if they are invited to submit full proposal – August 22, 2025
  • Full application due – September 26, 2025
  • Board meets – late October 2025
  • Applicants notified; funds delivered to successful applicants – Late November 2025

Criteria Information

Selection Criteria:

  • Organization is financially stable
  • Will have direct and/or potential to directly impact on patients, caregivers or patient care, or research that will contribute to knowledge in the field
  • Preference is for new initiatives or expansion of current programs
  • Preference will be given to organizations and projects that have a financial sustainability plan in place
  • Improving Access to Quality Care: Developing resources or programs that empower patients and caregivers to help them successfully navigate the healthcare system in order to access care and better manage the complexities of end-of-life care.
  • Advancing Research, Education, and Training: Creating tools to share new industry insights and findings to provide needed support to patients, caregivers, and health care providers.
  • Increasing End of Life Care Awareness: Empowering those facing serious illness or conditions and providing greater access to hospice and palliative care by bringing patients to the forefront of policy and regulatory conversations.
  • Unmet Needs & Emerging Solutions: Elea recognizes that innovative solutions may not always fit within predefined categories. Therefore, we welcome proposals that address unmet needs, emerging challenges, or transformative ideas that align with our mission. Applicants are encouraged to submit creative, evidence-based, or experimental approaches that demonstrate potential for meaningful impact. Funding decisions for this category will be based on the proposal’s alignment with our goals, feasibility, and anticipated outcomes.
  • Examples of these types of projects include:
    • Identifying and working to reduce barriers to coverage for care
    • Identifying quality in-home care opportunities
    • Supporting advanced care planning
    • Addressing health care inequities
    • Reducing barriers to earlier hospice election
    • Identifying solutions that enhance access and improve affordability
    • Identifying new, innovative patient-focused practices
    • Widely disseminating information on best clinical practices, including medical education of end-of-life care
    • Offering comprehensive education and training on best practices
    • Educational advocacy for legislative and regulatory relief/support
    • Leading multi-stakeholder conversations
    • Elevating key issues at a national level
    • Advocating for payment model reform
    • Supporting palliative focused program success in value-based payment arrangements
    • Supporting local and national communication and marketing efforts to advanced shared goals and advocacy efforts

Organizations that meet the eligibility criteria may apply for one grant in response to this RFP.  Organizations that have numerous entities or locations may submit only one application for each program area while specifying the location(s) of grant programming. Applications for special projects or operating support may be submitted.

Eligibility Criteria

  • Tax exempt under section 501(c)(3) of the Internal Revenue Code; and
  • Does work that is directly connected to the palliative and/or hospice care fields; and
  • Are registered and implementing programs within the United States; and
  • Prioritize increasing number of patients served and quality of patient care, where applicable.

Elea Institute does not consider applications from:

  • For-profit organizations, unless there is a specific charitable purpose with appropriate board oversight and expenditure responsibility
  • Organizations where project budgets include more than 25% of unrelated general operating costs
  • Grant funds cannot be used to directly support lobbying efforts
