The grant will be used to hire and train community health workers throughout a 13-county area of metropolitan Chicago. These front-line public health workers will offer education regarding hospice and palliative services and grief support focusing on historically underserved communities, and will also connect community members to resources pertaining to serious illness and social determinants of health.
