The HAP Foundation – Oakbrook Terrace, IL

The grant will be used to hire and train community health workers throughout a 13-county area of metropolitan Chicago. These front-line public health workers will offer education regarding hospice and palliative services and grief support focusing on historically underserved communities, and will also connect community members to resources pertaining to serious illness and social determinants of health.

The mission of The HAP Foundation is to improve hospice and palliative care through groundbreaking education and research. With a focus on individuals with serious illness and their families, it works to ensure a strong, equitable future for hospice, palliative care, and grief support, while also increasing access to these services through advocacy, education, workforce development, and research.
