2025 National RFP Grantees

Elea Institute's 2025 national request for proposals sought organizations advancing innovative, high-impact approaches to hospice and palliative care access. The following organizations were selected through a competitive review process.

Home Centered Care Institute (HCCI)
Expanding Access to Home-Based Healthcare by Integrating Two Care Models

Phase 1 of a four-year initiative to transform care for seriously ill, homebound individuals by integrating home-based primary care and home-based palliative care — two models that have historically operated in silos.

Increasing Access to POLST Conversations through Evidence-based Clinical Education

A two-year project to evaluate, refine, and disseminate a scalable training program that supports health care professionals in having serious illness conversations and translating patients' care preferences into POLST documents.

Massachusetts General Hospital & Johns Hopkins University
Hiding in Plain Sight: Who are those with serious illness who don't access ambulatory-based care?

A three-year research initiative to identify and better understand seriously ill older adults who need palliative or hospice care but remain largely invisible to health systems — rarely or never visiting a doctor's office. Led by researchers at MassGeneral Brigham and Harvard Medical School in collaboration with Johns Hopkins and Kaiser Permanente Northern California.
