Home Centered Care Institute – Oak Park, IL

The grant will pay for four Illinois-based networking and learning events during 2023. The purpose of these events is to engage and educate providers across the continuum of care, particularly home-based hospice and palliative care programs.

The mission of the Home Centered Care Institute (HCCI) is to increase patient access to home-based primary care as an innovative, cost-effective healthcare delivery model ideal for people with medically complex needs and limited mobility. Home-based primary care, or the “modern-day house call,” improves health outcomes for chronically ill, frail, elderly, and homebound patients by delivering accessible primary healthcare in the comfort of their home.
