Improving Access to Quality Care

Lightways Hospice and Serious Illness Care — Pediatric Expansion

Supporting Lightways' expansion of pediatric hospice services with resources for maintenance, equipment, and staffing, enabling more children and families to receive compassionate care at home.

Lightways Hospice and Serious Illness Care — Advance Care Planning

Funding caregiver training and advance planning resources to ensure families are prepared and supported throughout the care journey, helping patients remain in their preferred care setting.

Advancing Research, Education, and Training

Home Centered Care Institute — Innovation Awards Program

A partnership establishing an innovation awards program encouraging Illinois hospice organizations to implement chronic care management services and pilot new models of care delivery.

Lightways Hospice and Serious Illness Care — Music Therapy

Launching a music therapy program for pediatric patients, adding an important therapeutic dimension to end-of-life care for children and their families.

Illinois Hospice and Palliative Care Organization (IL-HPCO)

Expanding education services for hospice and palliative care professionals across Illinois, building a more skilled and supported workforce to serve patients statewide.

Loyola University Medical Center

Supporting the growth of Loyola's palliative care team and residency training programs, investing in the next generation of palliative care specialists and academic leaders.
